Rates
$500-650 Initial office visit for new patient (60-90 minutes) 

$400 Established patient comprehensive visit (40-54 minutes) 99215

$300 Established patient follow-up visit (30-39 minutes) 99214

$250 Established patient follow-up visit (20-29 minutes) 99213

$200 Established patient brief visit (10-19 minutes) 99212

In addition to office visits, it is our practice to charge on prorated basis for the time required to perform any other service which you may request of us including telephone conversations lasting more than 5 minutes.

Insurance
We do not bill insurance. However, you may want to consider submitting your bill to insurance for reimbursement. We can provide you with a receipt if you would like to file a claim with your health insurance plan. Our services may be covered in full, or in part, by your insurance. The insurer may ask for Dr. Bassler's NPI number, which is 1922153014.

Payment
Payment is due at the time of your service. Cash or check preferred.  We also accept most major credit cards.

Cancellation Policy
Services are by appointment only. Because your appointment time is reserved only for you, it is important that you do not miss any appointments. There is a 24 hour cancellation policy which requires that you cancel your appointment 24 hours in advance of your scheduled appointment to avoid being charged for the full visit. If you do not show up for your scheduled appointment, you will be required to pay the full cost of the appointment.
