I will strive to keep your information protected and confidential. My Electronic Medical Record (EMR) is Practice Fusion and is HIPAA-compliant. Practice Fusion is NOT accessible to providers at Mammoth Hospital or other facilities unless you provide them access through the patient portal.

Confidentiality & Privacy Policy

The law protects the relationship between a patient and a physician. Information cannot be disclosed by me without permission.

Exceptions include:

Those for I am required by law to report to the authorities.:

• Suspected child abuse or dependent adult or elder abuse
• Medical treatment by me for an injury caused by violence
• Threats to harm another person
• Threats by a patient to harm himself or herself

Standards of medical practice:

• Emergency conditions for which the health and safety of the patient warrant release of information without their permission.
• Consultations between treating providers.
• Peer review and quality assurance.
